Substitution Form for Attendees     

Registration
Registration includes three breakfast, two lunches, two receptions, the awards banquet, refreshment breaks, conference materials and access to session presentations.

When Is The Last Day to Register
? 

In order to ensure each attendee has a badge and meeting materials, registrations should be received no later than May 17, 2019.

Cancellation Refund Policy
All cancellations must be received in writing. Cancellations postmarked, faxed or e-mailed on or before May 17, 2019 will be refunded less a $100 administrative fee. No registration fees will be refunded after May 17, 2019. Any form received without payment will not be processed. Pre-registration is required by May 17, 2019, otherwise conference materials may not be available for on-site registrants.

Attendee Registration Fees
Early Bird Registration for Members (State or Corporate) - $700.00 per person - Closed April 15th
• Member (State or Corporate) - $800.00 per person
• Non-Member (State or Corporate) - $1,200.00 per person
• Additional Expo Staff - $700 per person
• Additional Sponsorship Staff - $700 per person
• Complimentary Exhibitor (1 per paid booth) - $0
• Complimentary Sponsor (based on sponsorship level) - $0


Louisiana State Employees Only
   • Monday Only - $300 per person
   • Tuesday Only - excludes the banquet - $300 per person
   • Sunday Reception & Monday Only - $300 per person
   • Monday & Tuesday - including banquet - $700 per person
   • Monday & Tuesday - EXCLUDING banquet - $600 per person

Spouse/Guest & Children Registration Fees

• Spouse/Guest - $400.00 per person
• Children 6 to 17 yrs old - $200 per person
• Children 5 yrs and younger - $0 per person


What Activities Can Spouses/Guest & Children Participate?
Registered spouses/guests and children can participate in the meal functions and social events. Spouse/Guest and Children registration includes the following events: President's Reception, Exhibitor Reception, 3 breakfasts, 2 lunches, and the Awards Banquet. Guest registration is defined as a spouse, significant other or other relative, it is not for co-workers of your organization. Spouse/Guest and Children are not permitted to participate in the presentation sessions during the day. Badges and/or tickets will be checked and collected at the functions prior to each event.
